b. Date of Birth.
2 Enclose a clear copy of the subject's valid U.S. Federal/State Government issued photo ID; e.g. driver's license, U.S. passport.
3. Enclose payment of $15.00 in the form of U.S. Money Order or U.S. Cashier's Check payable to the "Treasurer of Guam".
4. Enclose a self-addressed, stamped envelope so it can be mailed to yourself.
