Instructions to view PDF

In order to view or use PDF forms, you must have Adobe Acrobat Pro or Adobe Acrobat Reader (free) installed on your computer. Click here to download Adobe Acrobat Reader.

If you encounter problems opening a PDF in your browser, visit the below link for your browser information:

If you encounter the "Please wait..." message, you will need to do the following:

  1. Download and Save the PDF to your computer
  2. Navigate to the location where you saved the file (This is important. If you click on OPEN after the file is downloaded FROM YOUR BROSWER, you will continue to get the "Please wait..." message.)
  3. After navigating to the location, Double-click to open the file
